Chronic conditions

We plan safe exercise for people living with diabetes, heart conditions, arthritis and osteoporosis. Chronic means a health problem that lasts a long time.

What happens at your first appointment

We ask about your health, your medicines and what you want to be able to do. Then we check how far you can move, how strong you are, and what you can manage in a normal day. Most appointments take 30 minutes. If you have more than one problem, we book 45 to 60 minutes.

  • A plan built around the week you already have
  • We see you in the clinic, at the gym or at home
  • A Medicare care plan from your GP means these sessions are bulk billed
